Thank you for participating in the EFHCO hospital clown organization mapping survey.

This questionnaire has 20 questions and it will take you about 10-15 minutes to fill it out. The answers you give in the questionnaire will be shown in a data archive on the EFHCO website (www.efhco.eu/organizations). The data archive will be accompanied with a built in search engine which you can use to search for other organizations data.
